I am sure you have heard the warnings. Don’t lift heavy things. You might hurt the baby. It is the kind of advice that sounds responsible until you look at the actual evidence — which has been accumulating for years and now paints a very different picture.

The short answer: lifting weights during pregnancy, when done sensibly, is not only safe. It is one of the most beneficial things you can do for yourself and your baby.

What 50 Studies Tell Us

A recently published systematic review and meta-analysis pooled data from 50 studies covering more than 47,000 participants. The researchers looked at resistance training effects across the entire pregnancy timeline — prenatal health markers, delivery outcomes, pelvic floor symptoms, even mood.

Here is what they found:

  • Gestational hypertension (pregnancy-related high blood pressure) dropped by approximately 50% among those who lifted. This matters because uncontrolled hypertension during pregnancy can lead to serious complications for both mother and baby.
  • Gestational diabetes risk fell by roughly 38%. Resistance training improved insulin sensitivity in ways that matched or exceeded what we see from cardio alone.
  • Perinatal mood disturbances — anxiety and low mood during and after pregnancy — were reduced by about 50% across both the prenatal and postpartum periods. The physical work carried psychological benefits.

Here is what I tell my clients: if you have been told to stop lifting because someone worried it might cause problems, ask them which problem they are referring to. Because the data does not support most of the common fears.

The Risks People Worry About (That Did Not Increase)

This is the part that surprises most people. Resistance training did not increase rates of:

  • Preterm birth. The likelihood of delivering early was the same between exercisers and non-exercisers.
  • Caesarean delivery. C-section rates showed no meaningful difference.
  • Perineal injury (tearing in the genital area during childbirth). Again, no increase among those who lifted throughout pregnancy.

Birthweight and gestational age were unaffected. And interestingly, macrosomia — a medical term for a larger-than-normal birthweight that can complicate delivery — was actually less common among women who exercised regularly.

One small note: exercisers did tend to have a somewhat longer first stage of labor (the dilation phase). But there was no meaningful difference in the second stage (pushing), so it did not translate into harder deliveries overall.

Pelvic Floor Strength Matters

Limited but consistent data showed that women who maintained or increased their strength during pregnancy reported lower rates of pelvic floor symptoms — including incontinence and pelvic discomfort. The message is straightforward: a stronger pelvic floor going into labor is better than a neglected one.

Light Lifting Is Not the Only Option

The studies in this meta-analysis mostly used short sessions of light-to-moderate lifting, often combined with other physical activity. But heavier training works too.

A couple years ago, a study looked at women who continued intense Olympic weightlifting (snatch and clean and jerk) and CrossFit throughout pregnancy. The results were similarly promising — no increase in adverse outcomes, and the athletes reported better overall well-being compared to those who stopped training entirely.

Both of these papers — the earlier study on weightlifters and CrossFit athletes, and this meta-analysis — were led by Dr. Christina Prevett and Dr. Margie Davenport. They have done substantial work advancing what we know about training during pregnancy, and it is worth their names being known.

Day-to-Day Quality of Life

A 2025 systematic review of randomized controlled trials added another layer: women who resistance-trained during pregnancy reported measurable improvements in daily life. Less fatigue. Better sleep. Fewer aches and pains, particularly in the lower back and pelvis. And better physical functioning overall — which means carrying groceries, getting out of chairs, and doing the basic things that suddenly feel harder than they should.

How to Actually Do This

The research is clear on the whether. The how is where most people need practical guidance. Here is what I recommend based on the evidence and my coaching experience:

  • Keep sessions short. Most effective protocols in the studies ran 20 to 40 minutes. You do not need an hour-long gym session to get the benefit.
  • Prioritize controlled movements over max effort. Aim for loads at roughly 50-70% of your pre-pregnancy one-rep maximum. The goal is stimulus, not exhaustion.
  • Avoid holding your breath. Exhale on exertion. The Valsalva maneuver (bearing down hard while braced) puts extra pressure on the pelvic floor, which you do not want to stack on top of the 20+ pounds your body is already carrying by mid-pregnancy.
  • Skip exercises that put you flat on your back after the first trimester. Supine positions can compress major blood vessels. Use incline benches or switch to standing and seated variations instead.
  • If something feels sharp or wrong, stop. General fatigue is normal. Sharp pain, dizziness, or shortness of breath beyond what you expect — those are signals from your body, not challenges to push through. If you experience them regularly, consult a qualified professional before continuing.

The bottom line from the literature: lifting during pregnancy, when appropriately prescribed by a qualified professional, is not just safe. It is a net positive for nearly every measurable outcome we looked at — blood pressure, blood sugar, mood, pain, sleep, and how you feel on an ordinary Tuesday afternoon.

The Science, Simplified

When researchers pooled 50 studies with over 47,000 participants, the takeaway was straightforward: women who lift weights during pregnancy cut their risk of high blood pressure in half and diabetes by nearly 40%, while their mood improves, pain decreases, and sleep gets better. The risks everyone worries about — preterm birth, C-sections, tearing — did not go up. Start with light-to-moderate loads, keep sessions short, and build from there.
